5210 I Love Me
5210 promotes increasing fruits and vegetables consumption to five servings or more a day, reducing leisure screen time to 2 hours or less daily, increasing physical activity to 1 hour a daily, and eliminated sugar sweetened beverages. The 5210 in 30 Challenge initiated and further developed by local health professionals to include a self-monitoring tool to assist participants with adoption of lifestyle change over an initial period of 30 days.

Oregon Helps is a free and easy program prescreener. Answer questions about people in your house, income and expenses and this online tool will estimate your potential eligibility for various programs and assistance.
Service Integration Teams
This program is designed to coordinate community providers and services to identify needs, find solutions, and avoid duplication of services. Our partners for the Santiam Santiam Service Integration to include local school districts, law enforcement, local and state governments, faith communities, businesses, non-profits, early childhood providers, community volunteers, and other interested parties.
Bereavement Resources
Six-week grief support groups for adults are offered throughout the year. Participants gain support from others experiencing grief and loss as they gain an understanding of the grief process.
Community Health Needs
Santiam Memorial Hospital is using this system to conduct a comprehensive needs assessment, and will publish it in this location upon completion.
