Diabetes Education
The Diabetes Self-Management Education Program is designed to help patients learn to control blood glucose, which lessens risk of complications that can result from diabetes.
Diabetes is a chronic condition that requires patients to make multiple decisions throughout the day – decisions about food and medication, one must consider what to eat, when to eat, how to check blood glucose and how to manage high or low glucose, this education-based program is intended to provide patients the tools to modify behavior and successfully self-manage the disease and its related conditions.

Instruction is provided in a classroom format, and individual consultations are also offered.
The Diabetes Education Program at Santiam Hospital & Clinics is accredited through the American Association of Diabetes Educators. A doctor’s referral is required to participate.
